Subject:
Revision of application form for grant of Custom House Agent’s
License under section 146 of the Customs Act, 1962.

The Committee on Subordinate Legislation, Rajya Sabha had submitted its
recommendation in respect of Custom House Agents Licensing Regulations, 2004
on 8.3.2007. while the Committee had noted its satisfaction, in respect of
the changes notified by the Government vide notification No.99/2005-Customs (N.T.)
dated 7/11/2005, they desired to modify the Form ‘A’ i.e. the Application
Form for Grant of Customs House Agent’s Licence under the said regulations.
2.
Accordingly, the revised Form ‘A’ to be introduced taking into
account the recommendations made by the Committee is enclosed.
Views, comments and suggestions from the trade and industry
associations, departmental officers and from all other concerned are solicited
on the above revised Form. The views, comments and suggestions may kindly be sent before
30/6/2007 to the Under Secretary (Customs-VI), Room No. 227-A, North Block,
New Delhi – 110 001 (FAX No.: 23092173).

In response to the Public Notice No. ………… dated ……………, I/ we the undersigned hereby submit the following details for grant of Custom House Agent’s Licence under Section 146 of the Customs Act, 1962:
1. Name and full address of the applicant. (in case, the applicant is a firm or a company, the names and addresses of each of the partner of the firm or the director / managing director / manager of the company, as the case may be):-
2. Name and full address of the duly authorized employees, who will actually be engaged in the clearance of goods or conveyances work as Custom House Agent:-
3. In case it is desired to appoint clerk(s), the name and address of the clerk(s):-
4. Educational qualifications of each of the persons, who will actually be engaged in the work as Custom House Agent (including proof of working knowledge of computers):-
5. Particulars regarding knowledge of English, local language and Hindi (these particulars are required in respect of each of the persons actually engaged in the work as Custom House Agent):-
6. Particulars regarding knowledge of Customs Law and Procedure (these particulars are required in respect of each of the persons actually engaged in the work as Custom House Agent):-
7. Whether the applicant had earlier applied for a licence to act as Custom House Agent and whether such application was rejected.

Whether the applicant or the firm or company by whom he is employed
have earlier held a Custom House Agent’s Licence under these Regulations and
whether it was cancelled or suspended.
9.
Whether the applicant or any of the persons employed or proposed to be
employed by him have been penalized, convicted or prosecuted under any of the
provisions of the Customs Act, 1962 or any other law for the time being in
force:-

Number and date of each of the documents furnished in accordance with
regulation 5 of the Custom House Agent’s Licensing Regulations, 2004.
